PSYCHOSOCIAL REHABILITATION SERVICES (PSR)


Psychosocial rehabilitation (PSR) is a service that focuses on helping persons restore/attain the following skills: community living, personal care, social relationships, leisure time, education, and vocational skills. These skills are modeled, learned, and practiced in the community to ensure each member of the program is able to use the skills they’ve learned. Our PSR staff members are trained to provide a loving, caring, and supportive environment in which our members can flourish and achieve their goals. Our PSR program offers members lunch, social skills training, transportation, arts and crafts, and other education topics.

OUTPATIENT THERAPY SERVICES (OPT)


Outpatient therapy services are for those members of the community who suffer from emotional distress but are not eligible for inpatient services or other higher levels of care. Our outpatient service personnel are trained mental health professionals who are dedicated to helping others in their time of need. NuVizions’ staff can treat children, adolescents, adults, and the elderly.

CASE MANAGEMENT


Case management is a service that assists eligible recipients in gaining access to needed medical assistance as described in the North Carolina State Medicaid Plan, as well as to services not included in the State Plan. The goal of case management services is to facilitate the recipient’s medical, social, and educational needs. Case management includes the following core service components: assessment, care planning, referral/linkage, and monitoring/follow-up. The provision of this service requires the recipient to establish and maintain a medical home with Community Care of North Carolina (CCNC) or a primary care physician. CCNC or the primary care physician shall be responsible for the coordination of the recipient’s overall health care.

INDIVIDUALIZED PLACEMENT AND SUPPORT (IPS-SE)


Individualized Placement and Support (IPS-SE) services provide assistance with choosing, acquiring, and maintaining employment for individuals ages 18 and older for whom competitive employment has not been achieved and/or has been interrupted or intermittent. The primary outcome of SE is competitive employment, which is defined as a job that pays at least minimum wage, for which anyone can apply, and is not specifically set aside for people with disabilities.
